COMMUNITY HEALTH IMPROVEMENT PLAN​

Led by the Tulsa Health Department, more than 65+ community partners, created the 2023-2028 Community Health Improvement Plan (CHIP) for Tulsa County. The overall goal of the plan is to improve the health and well-being of Tulsa County residents in order to become the healthiest county in the United States.

The overall goal of the plan is to improve the health and well-being of Tulsa County residents in order to become the healthiest county in the United States. The CHIP is divided into three specific action plans to address the top three health concerns in Tulsa County:

  • Stress and mental health
  • Chronic disease risk factors and management
  • Healthy and affordable housing

Most importantly, this plan will address the social determinants of health: the conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning and quality-of-life outcomes and risks. Development for this plan began in late 2020 and continued through January 2023, with input from residents and stakeholders in the public and private sector. The plan is a long-term, systemic effort to address public health problems based on the results of community health needs assessment activities and the community health improvement process.

Make plans to attend quarterly meetings of the Tulsa County Community Health Improvement Plan (CHIP). Community partners across Tulsa County can come together to address data-identified health priorities, get involved and take action!

The Tulsa Health Department (THD) and the Oklahoma Public Health Association (OPHA) have partnered to strengthen community health through collaborative training, policy education and Continuing Education (CE) opportunities. Earn up to 2 CEs by attending a meeting. Together, they work to build public health capacity, empower community voices and advance equitable systems change across Tulsa County.

Meetings are held on Tuesdays from 9:00 – 11:00 a.m. in-person at the Tulsa Health Department North Regional Health and Wellness Center (5635 N Martin Luther King Jr Blvd, Tulsa, OK 74126) and online.

The CHIP Workgroups represents a cross-sector collaboration of community partners to address the three priority goals within the Tulsa County CHIP. We look forward to meeting and working alongside more experts in the field.

2026 Meetings Dates:

The year 2025 marks the first time the Tulsa Health Department has partnered with both Saint Francis Health System and Ascension St. John Medical Center to publish a joint Community Health Needs Assessment (CHNA). Conducted every three years, the CHNA identifies the most pressing health challenges facing Tulsa County. Importantly, the assessment amplifies the community’s voice drawing on the lived experiences, concerns and priorities of residents to ensure the findings truly reflect the needs of those who live and work in the area. These insights then form the foundation for all stakeholders to collaboratively develop strategic plans aimed at addressing these vital health issues.
