Back to School

The back to school season is a busy time for everyone, and the Tulsa Health Department is committed to helping make your life easier, happier and healthier!  Here is some helpful information about immunizations, shot records, and birth certificates.

Immunizations

Back-to-school immunizations are available on a walk in basis, no appointment necessary. Please bring a current immunization record and your insurance card.  For children’s immunizations, a parent or legal guardian must be present. For faster service, please fill out the Immunization Worksheet ahead of time and bring it with you. Childhood immunization clinics are available at the following times and locations:

Shot Records

Click here to request electronic shot records. Shot record requests can be made in person at any of the Tulsa Health Department locations or by phone at 918-595-4509. You can also fax a request sent to 918-595-4043.

 

Birth Certificates

For birth certificates visit the Vital Records department located at 5051 South 129th East Avenue, Tulsa, OK 74134. Birth certificates may be requested in person between the hours of 8:30 am- 4:00 pm at this location.  A valid photo I.D. along with payment in cash, check, or money order (made out to the Oklahoma State Department of Health). The cost is $15 per copy except when additional fees apply. As this is a busy time of year, wait times could be 1-2 hours. You can contact our Vital Records department directly at 918-594-4840.

 

Online OrdersVitalChek

We do not accept online orders directly; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service. VitalChek can be reached either through its website or by phone at 877-817-7364.  An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.
 

Immunizations Needed

Immunizing your infant or child with the current recommended childhood vaccines mobilizes the body's natural defenses against dangerous infectious diseases. Children through age 18 years are eligible to receive vaccines at no charge through the Vaccines for Children (VFC) program if any of the following apply: they are uninsured, Native American Indian, Native Alaskan, or their insurance policy does not cover vaccines. Infants, children and adolescents must be accompanied by their parent or guardian to receive immunizations.

THD currently accepts Cigna, Community Care, Blue Cross Blue Shield, Health Choice, Medicare and SoonerCare Medicaid for immunizations. Coverage can vary among different insurance plans. Please bring your insurance card and photo ID with you.  It is always advisable to check with your insurance provider for coverage specifics before receiving immunizations, as you may be responsible for charges that are not covered by your insurance policy. A fee will be charged for clients with private insurance (other than those listed above) to be immunized at THD clinics in order to cover the cost of the vaccine.
