Obtaining Birth and Death Certificates
Certified copies of birth and death certificates may be obtained at the Oklahoma State Department of Health's Tulsa Division of Vital Records located at THD:

Phone service is available between hours of 8:00 a.m. and 5:00 p.m.
Vital records are not open for public inspection. A birth certificate may be applied for by the person for whom it was issued, the immediate next of kin, or an authorized agent, who must provide a statement indicating that they are working in the best interest of the registrant, by written application, proper identification, proper information and applicable fees.
Any individual may obtain a death certificate upon written application, proper information and identification and applicable fees.
A valid, legal photo I.D. is required from the applicant or the individual representing the applicant for the issuance of a birth, death or stillbirth certificate. If mailing the request, please do not send the original ID with your application. Please send a photocopy.
Acceptable Identification (ID) of the individual applying for the birth certificate
A photocopy of a valid government issued photo ID by either the applicant or an individual attesting for the applicant is required for issuance of certificates of birth or death.
Acceptable current photo IDs are:
- State issued Driver's License
- State issued Photo Identification card
- U.S. Passport
- Government issued Military ID
- U.S. Bureau of Indian Affairs photo ID
- Tribal Identification card by federally recognized tribes with photo
- Oklahoma gun permit
- Pilot license
- Non-U.S. Passport and I-94 card (VISA)
- Consular Matricula card + 1 form of secondary ID
Please note the ID must be legible and must be current.
The fee for the search of a birth or death record is $15.00. This fee will pay for the issuance of one certified copy of the certificate to the qualified applicant if the certificate is located. Additional copies will be issued at a cost of $15.00 per copy. The searching fee is not refundable or transferable. Please make your check or money order payable to OSDH. Heirloom birth certificates are available for a fee of $35. Corrections or additions to vital records require additional fees and required documentation.
