Healthy Community Access Program

Improving the Flow of Health Care Access

The Healthy Community Access Program (HCAP) is a collaborative effort to improve the coordination of medical and dental care for the uninsured and underinsured in the Tulsa community. Central to HCAP’s mission is CareLink, a series of technology infrastructure systems that channel the flow of care in a continuum from prevention to tertiary care.

Inset - Community -HCAPCareLink includes:
NurseLink - a telephone-based nurse triage system that helps the uninsured and underinsured in our community.

ShareLink - an information network for local health programs and agencies to help coordinate overall patient care.

HealthLink - a centralized appointment system that works in conjunction with BabyLine, PlanLine, and KidsLine. 4-HEALTH (918-443-2584)

HCAP also conducts community health needs assessments and other studies to determine health patterns and identify barriers to health care for people in our community.

Recent HCAP studies such as the most recent Community Health Needs Assessment and the Emergency Room Utilization Study in partnership with OU’s College of Public Health.
